Hunter armory. Ignore the gear and check the achievements. Specifically check the date of the Onyxia achievement. Compare that to when Onyxia level 80 came out.

Why? The quest popped up the moment that you had done the head. He was maybe pulled through, had been given the head, and has gotten the quest that way?
Oh. I didn't even think of that. The actual process was different. This is now a guessing game: How did this happen?

He doesn't have the BWL achievement.
what might have happened was that he turned in an old quest? He could be a packrat with an old quest for turning in Onyxia's head. There have been reports of people gaining the feat of strength for obtaining the Aqua Quintessence.
Shy beat you to it. He wasn't a packrat. I'll give you a hint: He did MC a week or so before.
My hunter got the leave from MC and did the epic chain. The sinew from Onyxia now drops off drakes in the Burning Steppes, presumably so that a level 60 quest doesn't require a level 80 raid. The result trivializes that aspect of the chain.
